HOSPITAL REFERRAL FORM
If you need an ultrasound, please have your veterinarian fill out the hospital’s referral form and send it to harpersounds.vet@gmail.com.
If it is an emergency or your pet needs to be seen immediately, please have your vet email harpersounds.vet@gmail.com and check the schedule and call the hospital she is working at that day and/or email harpersounds.vet@gmail.com
